Baron "Bear" Bailey has romantic feelings for Nikki Freeman, his childhood friend and co-worker at a local music store. While shopping for a gift for her at a mystic shop, Bear buys a "One Wish Willow”, a novelty toy that claims to grant one wish per person when broken.

That night, after Bear drops her off at her house, Nikki directly asks him if he likes her, which he denies out of fear. Frustrated with himself, Bear breaks the One Wish Willow, wishing for Nikki to love him more than anyone else in the world. Nikki reappears and asks to sleep at Bear's house, revealing that her estranged father is dying of cancer. In Bear's bed, she begins to kiss him, but abruptly screams and distances herself, before snapping back.

In the morning, Nikki creates a memorial with the remains of Bear's cat Sandy, who died after accidentally consuming oxycodone, disturbing Bear. She later explains her erratic behavior as a result of taking MDMA and admits her feelings for Bear. The two become a loving couple, to the befuddlement of their friends Ian and Sarah.

While at a restaurant, Bear receives a call from Ian, who informs him that Nikki expressed disinterest in Bear shortly before they got together and lied about her father's cancer. Upon being confronted, Nikki becomes upset until Bear promises not to mention it again. That night, Nikki watches him sleep, moves strangely around the room, and accuses him of not loving her as much as she loves him. The next day, Nikki covers the entire front door with duct tape. Despite this, Bear still leaves for work, where he vomits upon discovering Nikki has prepared him a sandwich made out of Sandy's remains.

Bear calls One Wish Willow's customer support, hoping to alter his wish. The voice on the other end states that the wish will only expire when Bear or Nikki dies, then puts a hysterically screaming Nikki on the line. At home, Bear discovers Nikki, covered in vomit and bodily waste, standing in the same position he left her in.

Despite Bear and Ian's protests, Nikki accompanies Bear to a party at Ian's house. During a game of drunk Jenga, Nikki recites a violent and incestuous retelling of Hansel and Gretel, horrifying the guests. Bear draws a block ordering him to kiss the person to his left, which is Sarah. Nikki ousts Sarah from her seat and kisses Bear instead, then suddenly begins screaming and stabbing her own face with a broken bottle. Later that night, Nikki threatens suicide to make Bear join her in bed. Sarah texts Bear, asking him to meet her. While Bear attempts to leave, a lucid Nikki awakens, claims her obsessive persona is asleep, and pleads with Bear to kill her before her obsessive side wakes up. Bear refuses, offended that she would rather die than remain in a relationship with him.

Outside, in her car, Sarah reveals Ian and Nikki were in a casual sexual relationship for years before hinting at her own attraction to Bear. Nikki appears and repeatedly slams Sarah's head into a brick against the steering wheel, killing her. Nikki reminds Bear that this is what he wished for, and instructs him to go home while she disposes of Sarah's body. Bear returns to the mystic shop and buys another One Wish Willow, but is unable to break it. He explains everything to Ian and begs him to undo the wish on his behalf. Ian, disbelieving him, instead wishes for a billion dollars. Cash immediately rains from his ceiling, to Bear's chagrin.

Bear returns home with another One Wish Willow, planning to ask Nikki to reverse his wish. He sees Sarah's mangled corpse sitting in a chair completely nude before Nikki appears wearing her clothing and threatens Bear and herself at gunpoint. Ian arrives to celebrate his newfound wealth, but Nikki immediately shoots him dead. Bear locks himself in the bathroom with the gun and deliberates shooting himself, ultimately choosing to instead swallow the same oxycodone that killed Sandy. He seemingly changes his mind and attempts to make himself vomit. Before he can do so, Nikki uses the final One Wish Willow. Bear, now possessed, emerges from the bathroom and kisses her before overdosing and dying. A heartbroken Nikki prepares to shoot herself, but suddenly regains control of herself and starts wailing upon seeing the carnage around her.
